Valid health insurance card (RAMQ for residents of Quebec) or other valid insurance (Blue Cross or other).
Referral letter to the PrévOnco Clinic from your healthcare professional.
Previous test results or medical reports (if available)

Cancellation Policy

24+ Hours
24-hour notice: Appointments can be cancelled or rescheduled without charge
< 24 Hours
Less than 24 hours: A cancellation fee may apply
No-Show
No-show: Full appointment fee will be charged
Emergency situations will be considered on a case-by-case basis
